• You must be 18 years or older and a resident of the United States of America to join MedeliaMonitor™ Community of Women.
• We reserve the right to deny or revoke membership in the MedeliaMonitor™ community of women.
• One Membership Form application per individual.
• Individuals must be current members of the MedeliaMonitor™ Community of Women and email entered must match email address on file when responding to surveys to be eligible for awards.
• We will never share your personal information.
• From time to time we will offer incentives that must be mailed. We promise to inform you that shipping information will be required before you fill out a survey.
• Incentive offered for surveys or referral programs cannot be substituted by other gifts or cash.
• Checks will be issued for cash incentive programs.
• Members are responsible for cashing checks in a timely manner.
• The cost of canceling check will be deducted if a new check must be issued.
• Employees or relatives of the MedeliaMonitor™ and its parent company Medelia Communications are not permitted to participate in or receive awards.
• Award winners are required to give permission to publicize their full name, city, state and photo on the MedeliaMonitor website and all printed materials.
• The MedeliaMonitor™ is a work in progress. There will be additions to our policy as we learn and grow.
